SMS Messaging Service
By providing my phone number, I consent to receive SMS text messages from Connect Physical Therapy and Wellness, Inc. for appointment reminders, marketing messages, and general two-way communication about your relationship with Connect Physical Therapy and Wellness, Inc. Message and Data Rates may apply.
Message Frequency
You will get more than one message from us unless you opt-out, and while messaging frequency varies, you will likely receive messages pertaining to your account with us as you request, or up to 4 messages per month In addition, you will receive text message confirmations for your appointments if you do not opt out. Connect Physical Therapy and Wellness, Inc. reserves the right to alter the frequency of messages at any time to increase or decrease the total number of messages. Connect Physical Therapy and Wellness, Inc. and carriers are not liable for delays or undelivered messages.

Cancellation/Opt-Out Instructions

Health Information Disclosure
Text messaging is not a secure or encrypted method of communication. By opting in, you acknowledge that SMS text messages are not compliant with HIPAA security standards, and there is a risk that health information shared or received via text could be intercepted, viewed, or accessed by unauthorized parties. Connect Physical Therapy and Wellness, Inc. will not send sensitive protected health information (PHI), such as diagnoses, treatment details, or clinical notes, via text message. Text messages from Connect Physical Therapy and Wellness, Inc. will generally be limited to appointment reminders, scheduling, and general administrative communication.
Appropriate Use of Text Messaging
Text messaging with our office should not be used to communicate detailed clinical information, request prescription refills, report a change in condition requiring immediate attention, or communicate any information you consider urgent. Please call our office directly for these matters.
Medical Emergencies
Text messaging is not monitored 24/7 and should never be used in the event of a medical emergency. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not attempt to reach us via text message for urgent or emergency medical needs.
Liability
We are not responsible for any charges, errors, or delays in SMS delivery or undelivered messages caused by your carrier or third-party service providers.
